Please obtain a summary of your NHS medical record(containing a full list of diagnoses, medications and recent blood tests) from your NHS GP along with any copies of repeat prescriptions and relevant consultant letters.

Please bring all of these items with you to your appointment with a copy of the form you require completing. This is often the D4 medical form which can be found online. We can print a copy of the D4 form at the clinic if you require.
