How to prepare for a D4 Driver Medical

To ensure you get your completed form on the day, please prepare in advance by following these steps:

  1. Obtain a summary of your NHS medical record from your NHS GP, including a complete list of diagnoses, medications, and recent blood tests. Alternatively, if you have the NHS app that will provide the relevant information.
  2. Bring copies of any repeat prescriptions and relevant consultant letters.
  3. If you are over 60, it is advisable to have had a recent (within the past year) health check with your doctor or GP.
  4. Download and print the D4 medical application form from the official website: Medical examination report for a lorry or bus driving license (D4).

Do you need an extended medical?

If you are diabetic or have more than two past medical health problems, you will need to book an extended Driver Medical.

Diabetics will need to bring their most recent three month continuous history of glucose readings with them to the appointment otherwise they will not pass the medical.

No, the medical examination is the same for all "large vehicle" categories and taxis.

How long does a D4 medical appointment take?

A standard Driver Medical appointment takes 20 minutes. Patients with two or more health complaints or diabetes must book an extended 40-minute appointment.

What if I can't access a printer to print the D4 form?

We can provide you with a printed copy of the D4 form at the clinic if needed.
